    A new model for primary care multi-disciplinary teams has been developed which is seeing local GP practices focus not just on managing illhealth, but also on the physical, mental and social wellbeing of communities.

    Practice-based physiotherapists, social workers and mental health practitioners, are now based in a number of GP practices.

    These new teams are being supported by increased investment in district
    nursing, health visiting, training, and access to appropriate specialist advice.

    Enhanced teams are now been rolled out across Down, Derry/Londonderry and West Belfast GP Federation areas.

    These areas account for approximately 365,000 people who will potentially
    benefit from this new model of care.

    One of the practices involved in this roll out is headed by Killyleagh GP Dr Ultan McGill who explains below how the model has been embraced by patients and clinicians alike.
