Progress Report- Progress against our ambitions

To deliver on the four aims set out within Delivering Together, and ultimately to support people to lead long, healthy and active lives, we committed to changing the focus of our services, and importantly, how and where those services are delivered.

To do this we committed to:

• Build capacity in communities and in prevention.
• Provide more support in primary care.
• Reform our community and hospital services.
• Organise ourselves to deliver.
