This Pharmacy Incident Survey form allows the accurate assimilation of data pertaining to robberies in phamacies throughout Northern Ireland.DoH would ask pharmacies that have been affected to complete the form below for all such incidents dating from 1 January 2011 to date.The completed form can be submitted electronically to mrg.survey@health-ni.gov.uk
