A Framework for Personalised Care and Population Health for Nurses, Midwives, Health Visitors and Allied Health Professionals

This framework has been developed to underpin our national programme to maximise the impact of nurses, midwives, health visitors and allied health professionals (AHPs) on improving health outcomes and reducing inequalities.It aims to support and shape “health promoting practice” that encompasses both personalised care and population health across all ages, care places and with individuals, families and communities.
