Introduction
There are four key aims that embeds the ambition of Health and Wellbeing 2026: Delivering Together
Our ambitions
To deliver on the four aims set out within Delivering
Together, and ultimately to support people to lead
long, healthy and active lives
Building capacity in communities and in prevention
To reduce inequalities and ensure the next generation is healthy and well.
Providing more support in primary care
To enable more preventative and proactive care, and earlier detection and treatment of physical and mental health problems.
Reforming our community and hospital services
To support everyone to lead long, healthy and active lives, we need to change the focus of how and where our services are delivered.
Organising ourselves to deliver
The Health and Social Care Board (HSCB) closed on 31 March 2022 and responsibility for its functions transferred to the Department of Health.
